After leaving the hospital pharmacy, they were involved in a minor crash before continuing along Flemington Road, he said.
A few hundred metres later, Senior Sergeant Clanchy said the woman appeared to lose control of the car before slamming into a tramway pole at speed.
“It would have been a very tragic event for people to have witnessed. Unfortunately, there was nothing anybody was able to do … it happened very quickly; the vehicle was engulfed in flames very quickly as well,” he said.
The cause of the crash is yet to be determined, however police have ruled out the incident being a terrorist act. Investigators are preparing a report for the coroner.
A friend of the family said in an online post that her heart was “shattered” by the death of Ms Poole, who she described as a happy, kind and funny person.
“You were thoughtful, and my biggest supporter. It is hard to find someone as loyal and loving as you,” the woman wrote.
“There wasn’t a day that went past that we wouldn’t talk … I will never fully recover from this.”
